Service provided under the CIH Waiver. RHS services provide up to a full day (24-hour basis) of services and/or supports that are designed to ensure the health, safety, and welfare of the participant. This service may be provided at an hourly rate or daily rate, dependent upon the individuals ALGO score.
Service provided under Family Supports Waiver (FSW). PAC services are provided in order to allow participants with intellectual & developmental disabilities to remain and live in their own homes and to participate in their communities. These services support and enable the individual in activities of daily living and self-care with hand
Service provided under Family Supports Waiver (FSW). PAC services are provided in order to allow participants with intellectual & developmental disabilities to remain and live in their own homes and to participate in their communities. These services support and enable the individual in activities of daily living and self-care with hands-on support and monitoring to ensure the health and safety, of the individual.
Service provided under FSW and CIH Waiver. Respite care means services provided to individuals unable to care for themselves that are furnished on a short-term basis because of the absence or need for relief of those persons who normally provide care for the individual. Respite care can be provided in the individual’s home or place of residence or in the respite caregiver’s home.
Services provided outside of the participant’s home that support learning and assistance in the areas of self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills.
Wellness Coordination services means the development, maintenance, and routine monitoring of the waiver participant’s Wellness Coordination plan and the medical services required to manage his or her health care needs. This includes Tier 1, Tier 2 and Tier 3 wellness services.
WHAT DOES THIS MEAN? Achieving accreditation requires a service provider to commit to quality improvement, focus on the unique needs of each person the provider serves, and monitor the results of services that are provided.The accreditation process begins with an internal examination of its program and business practices. The provider then requests an on-site survey that is conducted by a team of expert practitioners selected by CARF. During the survey, the provider must demonstrate that it conforms to a series of rigorous and internationally recognized CARF standards. Based on the results of the survey, CARF prepares a written report of the provider’s strengths and areas for improvement. If a provider has sufficiently demonstrated its conformance to the standards, it earns CARF accreditation
